Evidence of Insurability

Proof of good health, also known as evidence of insurability (EOI), is an application process in which you provide information on the condition of your health and/or your dependent’s health to get certain types of insurance coverage. 

As a new hire, you can elect up to the guaranteed issue (GI) without EOI. For optional employee life insurance, the GI is the lesser of $250,000 or 5x your benefit salary. For your spouse or domestic partner’s life insurance, the GI is the lesser of $30,000 or 50% of your optional employee life insurance. Amounts elected above the GI will require EOI. (If you elect optional life coverage, you will be enrolled with $250,000 or $30,000 until the EOI is approved.)

If you are enrolled in at least $10,000 but less than $250,000 of optional employee coverage, you will be able to enroll up to $250,000 without EOI after a Qualified Life Event or during annual Open Enrollment. Any amount over $250,000 will require EOI.  If you are not enrolled in any optional employee coverage, you will be required to submit EOI for any coverage amount elected after a Qualified Life Event or during annual Open Enrollment. 

If you are enrolled in at least $5,000 but less than $30,000 of optional spouse coverage, you will be able to enroll up to $30,000 without EOI after a Qualified Life Event or during annual Open Enrollment. If you are not enrolled in any optional employee or spouse coverage, you will be required to submit EOI for any coverage amount elected after a Qualified Life Event or during annual Open Enrollment.

If you make an election that is subject to EOI, you can complete your EOI online or by completing a paper medical history form (please open using Internet Explorer). Please review the instructions for EOI submission (PDF) for the steps involved in the online EOI process. You have 60 calendar days from your hire date or Qualified Life Event date to complete your EOI. Please have company code GWUEE available when completing your EOI. You will need the company code to register on the website.
